The information requested within the referral form covers the following areas:

Step Three - Assessment
Our assessments provide a quality of service to ensure that we have all the necessary information to formulate a programme that will benefit the child or young person. It also provides a chance for first contact, a chance to see the proposed site, so that expectations are met for all stakeholders. An initial site visit and assessment visit will be arranged for the applicant, parents-carers and any other stakeholders as deemed appropriate.
Step Four - Review
The Senior Management team will discuss and review the existing assessment information for the child or young person and build a programme based upon the information received, provided and formulated from the meeting.
Step Five - Agreed Duration
Dependent on the ideal placement period and number of days required per week, a trial period of 2 days or longer may be agreed upon. This will precede the agreed overall duration as part of a contracted agreement.
Final Steps
If the AP School feel they can meet the applicant’s needs, the referrers agree terms and a place will be offered subject to availability.
